Submitting Dependent Requests

Use the Dependent Change Request page to submit a request to update your electronic personnel file by adding a dependent record, changing a dependent record, or removing a dependent record.

Adding a Dependent

In the Demographics Request Queue, click the New Request link to submit a request to add a dependent to your electronic personnel record.

To submit the request:

  1. Click the Request Changes link on the Home page. The Demographic Requests page displays.
  2. Click the Request Type Drop-down List button and select Dependent from the list.
  3. Click the New Request link to display the Dependent Change Request page. If applicable, the Dependents grid lists the dependent records that are part of your personnel record.
     

    The message displayed in the General Information area is entered by the administrator. This message may change so it is important to read the message each time you make a request.

  4. Click the link above the Dependent grid to display the Dependent Change Request page with all fields blank.
  5. Enter the dependent information. The following fields are required: First Name, Last Name, Birth Date, Relationship, and Gender.

  6. In the Comments area, type comments to the approver about this request. You can enter a maximum of 246 alphanumeric characters.
  7. Click the button to submit your request. You will receive an automated e-mail that contains the request details and its current status (pending).

    The request is automatically routed to the first approver in the approval chain.

Changing a Dependent Record

Click a Dependent Name link in the Dependents grid to submit a request to change a dependent's record.

To submit the request:

  1. Click the Request Changes link on the Home page. The Request History grid displays.
  2. Click the Request Type Drop-down List button and select Dependent from the list.
  3. Click the New Request link.
     

    Note: The message displayed at the bottom of the page is entered by the administrator. This message may change so it is important to read the message each time you make a request.

  4. In the Dependent grid, click the Dependent Name link that corresponds to the dependent record to be changed. The Dependent Change Request page displays the details currently on file for that dependent.
     

    Note: The message "Relationship Type [Relationship type Value] no longer exists. Please select a valid relationship before submitting the request." displays if the current Relationship Type assigned to the dependent record is not valid or is inactive. Click the  button to close the message. Click the Relationship Drop-down List button  to select the relationship type that describes the dependent's relationship to you.

  5. Make the necessary changes to the information displayed. Click here for a description of each field.
  6. In the Comments area, type comments to the approver about this request. You can enter a maximum of 246 alphanumeric characters.
  7. Click the button to submit your request. You will receive an automated e-mail that contains the request details and its current status (pending).

    The request is automatically routed to the first approver in the approval chain.

Submitting a Request to Delete a Dependent Record

Use the Delete button to submit a request to remove a dependent from your electronic personnel file.

  1. Click the Request Changes link on the Home page. The Request History grid displays.
  2. Click the Request Type Drop-down List button and select Dependent from the list.
  3. Click the New Request link.
  4. Click the Delete button that corresponds to the dependent record to be removed. The Create Delete Dependent Request page displays.
  5. Optionally, type the reason for removing the dependent from your personnel record. You can enter a maximum of 246 alphanumeric characters.
  6. Click the button to submit your request. You will receive an automated e-mail that contains the request details and its current status (pending).

    The request is automatically routed to the first approver in the approval chain.
