Emergency Contacts Request Field Definitions

You can enter or change the following fields when submitting an Emergency Contact request. This information is stored in your electronic personnel record in LINQ ERP.

The Additional Comments and Current Comments fields display only if the LINQ EP administrator grants permission to submit an emergency contact request that will change the comments currently stored in LINQ ERP.

 

Field Definition Req

Name

Enter/change the name of the emergency contact. You can enter up to 40 alphanumeric characters.

Relationship

Click the Drop-down List button to select the relationship type that describes the emergency contact's relationship to you, e.g., spouse, son, daughter, etc.

Address

Enter/change the first line of the emergency contact's address. You can enter up to 40 alphanumeric characters.

City

Enter/change the name of the city where the emergency contact's address is located. You can enter up to 40 alphanumeric characters.

State

Click the Drop-down List button to select the state where the emergency contact's address is located.

Zip CodeType the 5- or 9-digit zip code for your primary mailing address.

Enter/change the zip code associated with the emergency contact's address. You can enter a 5-digit zip code or the Zip + 4 without punctuation (i.e., 999994444).

Phone 1

Enter/change the primary telephone number for this emergency contact without punctuation (i.e., 1234567890).

Phone 2

Enter/change a second telephone number for this emergency contact without punctuation  (i.e., 1234567890).

Additional Comments

Enter comments about this emergency contact to be added to the employee's emergency contact record in LINQ ERP.  The program determines the maximum number of characters based on the number of characters already entered in the emergency contact comments in LINQ ERP.

If comments already exist (see the Current Comments field), you must start your comment with a semi-colon (;) or space. When the emergency contact request receives final approval, this punctuation mark is used as a separator between the added comment and the current comment when it is appended to the current comment in LINQ ERP.

This field displays comments about this emergency contact that are currently stored in LINQ ERP.
