Touring the Dependent Page
Click the Dependents link to list your dependents as entered in LINQ ERP and their related insurance coverage plans.
You can click a Name link to view the dependent's insurance coverage details, if available.

Click this link to submit a request to add a dependent or change a current dependent's record in LINQ ERP. |
Dependents Grid Field Definitions
The Dependents grid displays the following information sorted by name. Optionally, click the Filter button 
to display a filter box for each column. Use these filter boxes to search for specific dependent records.

Name |
This column displays the name of each dependent in blue text. You can click the name to view that dependent's insurance coverage details. |

Address |
This column displays each dependent's mailing address. |
|
Relationship |
This column displays the employee's relationship with each dependent. |
|
Gender |
This column displays the gender of each dependent. |
|
Birth Date |
This column displays each dependent's date of birth in mm/dd/yyyy format. |
|
SSN |
This column displays each dependent's Social Security Number, if available. |
|
Phone No |
This column displays each dependent's phone number. |
|
Coverage Start Date |
This column displays the date each dependent's current insurance coverage began. |
|
Coverage End Date |
This column displays the date each dependent's insurance coverage stopped. |
|
Student |
N indicates the dependent is not a student. Y indicates the dependent is a student. |
